USA Sport Group
All online payment transactions are processed securely through USA Sport Group LLC on www.USAsportGroup.com using Authorize.net as the dedicated payment gateway on behalf of United Soccer Academy Inc (herein “USA”) and the US Sports Institute Inc (herein “USSI”). I agree that I am entering an agreement with USA and or USSI to provide me with products or services and understand that USA Sport Group LLC and Authorize.net are handling my secure transaction and are not in any way responsible or liable for the service provided to me by USA or USSI and that my credit card statement will detail payment transaction through USA Sport Group. I understand that if I have registered for a Soccer or Soccer Squirts program that United Soccer Academy, Inc will conduct this program, and if I have registered for any other sports program, it will be administered and conducted by the US Sports Institute, Inc.

Terms of Use
By accessing or using our web sites at www.unitedsocceracademy.com, or ussportsinstitute.com, or soccersquirts.com or www.usasportgroup.com (together the "Site") you (the "User") signify that you have read, understand and agree to be bound by these Terms of Use ("Terms of Use" or "Agreement"), whether or not you are a registered member of United Soccer Academy, Inc; US Sports Institute, Inc or USA Sport Group LLC on our Site. We reserve the right, at our sole discretion, to change, modify, add, or delete portions of these Terms of Use at any time without further notice. If we do this, we will post the changes to these Terms of Use on this page and will indicate at the top of this page the date these terms were last revised. Your continued use of the Service or the Site after any such changes constitutes your acceptance of the new Terms of Use. If you do not agree to abide by these or any future Terms of Use, do not use or access (or continue to use or access) the service or the Site. It is your responsibility to regularly check the Site to determine if there have been changes to these Terms of Use and to review such changes.

Registration
In consideration of your use of the Site, you agree to (a) provide accurate, current and complete information about you as may be prompted by any registration forms on the Site ("Registration Data" or “Family Profile”); (b) maintain the security of your password and identification; (c) maintain and promptly update the Registration Data, and any other information you provide to keep it accurate, current and complete; and (d) be fully responsible for all use of your account and for any actions that take place using your account

Terms of Sale
Certain products and services are made available to you ("you" or "User") by USA, USSI or its corporate affiliates (collectively, "us", "we" or "the Company"). By purchasing any products or services you signify that you have read, understand and agree to be bound by the Terms of Sale in effect at the time of purchase ("Terms of Sale"). These Terms of Sale are subject to change without prior written notice at any time, in our sole discretion, and such changes shall apply to any purchases made after such changes are posted to the Site. Therefore, you should review these Terms of Sale prior to each purchase so you will understand the terms applicable to such transaction. If you do not agree to these Terms of Sale, do not make any purchases with USA or USSI.

Refund Policy
Refunds will only be made if cancellation occurs two weeks prior to the start of the event and will incur a $50 administration fee for all cancelled registrations. Alternatively a credit voucher for the full amount will be issued if desired. Cancellations within two weeks of the start of the event are only entitled to credit vouchers unless a medical emergency arises at which point documentation is required from a medical authority. Refunds will not be issued in the event of non-participation. No refunds or credits will be issued once a program or camp has started.

Purchase Qualifications; Account Security
To make an online purchase with USA or USSI, you must be a Registered User and comply with these Terms of Sale (including the Terms of Use). You acknowledge that you are responsible for maintaining the security of, and restricting access to, your account and password, and you agree to accept responsibility for all purchases and other activities that occur under your account. USA and USSI sells their online services and products only to those users who can legally make purchases with a credit card. USA and USSI reserves the right to refuse or cancel orders or terminate accounts, at any time at their sole discretion.

Payment Methods
We accept Visa, MasterCard, American Express and Discover credit cards plus personal checks as forms of payment. By submitting an order through our Sites, you authorize USA, USSI or its designated payment processor, to charge the account you specify for the purchase amount. All payments are to be made in United States Dollars. When you provide your credit card number to us, you authorize us to store your credit card number and related payment information. If you are paying online by check your order will be taken as pending until your check is received and processed at which time you will then receive a confirmation email only if you have provided a valid email address during registration. When paying by check your place is not confirmed in the program until the physical check has been received by USA, USSI or USA Sport Group.

Discounts and Special Offers
USA and USSI offer the following discounts and special offers to its customers for promotional purposes. We reserve the right, at our sole discretion, to change, modify, add, or delete portions of these discounts and special offers at any time without further notice.

1. Gold Club Membership
Gold Club Membership must be purchased at a value specified in the shopping cart pages under the “Terms of Sale”. Once purchased, Gold Membership has zero cash value and cannot be refunded or credited in any way. You must be a registered user of the Site to purchase a Gold Club Membership, which once confirmed, entitles the customer the following discounts on a lifetime basis or as long as they remain a Registered User. Gold Club discounts do not carry across various children; the percentage of the discount received is relative to the total camps registered for by one child in a single transaction. However, each family need only purchase one Gold Club Membership which can then be used to receive discounts on each child individually.
10% off when a child registers for 1 camp.
15% off when a child registers for 2 camps.
20% off when a child registers for 3 camps.
25% off when a child registers for 4 or more camps.

2. Gift Coupons and Promotional Codes.
USA and USSI will issue gift coupons and promotional codes for certain products, services or to specified Registered Users on either a dollar or percent value. Purchase of gift coupons must be made based on the “Terms of Sale”. It is the responsibility of the customer to enter correctly any coupon codes during the shopping cart pages and or registration forms to be entitled to the discount or special offer. We reserve the right to refuse all and any use of promotional codes for misuse.

Weather Policy

If a portion of the class is cancelled by USA or USSI due to adverse weather conditions we will do it’s best to make up all lost time during the camp week or session. Make ups will not take place during any other week or session. If total time lost to inclement weather is greater than one full camp day or class and USA or USSI are unable to make the time up then a pro-rated credit voucher will be issued for lost time. Credit vouchers will NOT be issued for any lost time that is less than one full camp day or session. Cash refunds WILL NOT be issued in the case of weather cancellations. In case of inclement weather campers should check our websites for information and if in doubt, arrive on site.

Player / Camper Information
USA Summer Camp price includes a T-shirt, soccer ball and certificate unless otherwise stated. USSI Summer Camp price includes a T-shirt and certificate unless otherwise stated. Soccer Squirts Summer Camps price includes a T-shirt, soccer ball and certificate unless otherwise stated. USA and Soccer Squirts Campers should bring a soccer ball to camp and wear shinguards. All campers should bring enough water to last the duration of their programs and snacks and/or lunch as their individual program indicates. No T-Shirts or Soccer Balls are issued on any camps, classes or programs outside of the summer season.

Waiver and Hold Harmless Agreement - United Soccer Academy, Inc.

When you register with United Soccer Academy, Inc. directly or via www.USAsportGroup.com you agree to the following:

You agree to list all additional medical information by enclosing an additional written documentation when registering.
I hereby agree to let my child participate in the sport of soccer. I understand that there are certain risks of injury inherent in the practice and play of this sport, as well as in traveling and other related activities incidental to my participation, and am willing to assume these risks. I hereby certify that my child is fully capable of participating in the sport of soccer and that he/she is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in this activity, except as included in writing with this application. In addition to giving full consent for my child's participation, I do hereby waive, release and hold harmless United Soccer Academy, Inc, its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the sport of soccer and the activities incidental thereto, whether the result of negligence or any other cause. I grant permission for my child to receive emergency medical treatment. I grant United Soccer Academy, Inc permission to use photographic or video images of my child in future promotional materials.

When you register with US Sports Institute, Inc. directly or via www.USAsportGroup.com you agree to the following:

I understand that there are certain risks of injury inherent in the practice and play of this sport, as well as in traveling and other related activities incidental to my participation, and am willing to assume these risks. I hereby certify that my child is fully capable of participating in the sport and that he/she is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in this activity, except as included in writing with this application. In addition to giving full consent for my child's participation, I do hereby waive, release and hold harmless US Sports Institute Inc, its officers, coaches, sponsors, partners, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the sport and the activities incidental thereto, whether the result of negligence or any other cause. I grant permission for my child to receive emergency medical treatment. I grant US Sports Institute, Inc permission to use photo or video images of my child in future promotional materials.
